Job summary

Health Connect America, Inc. is seeking an Independent Contractor-Family Support Services professional to help families improve functioning and child well-being.

You will coordinate with community providers, participate in collaborative case planning, and deliver in-home case management and visitation support. Deliverables include timely logs, progress updates, and accurate documentation aligned with billing and reporting standards.

Qualifications

• Bachelor’s degree in a human services-related field.
• Or High School Diploma/GED with at least 5 years of verifiable experience in the field.
• Must have reliable transportation, valid driver’s license, and proof of insurance.
• Willingness to complete required background checks and contractor onboarding

Responsibilities

• Coordinate with community providers to facilitate comprehensive care and referrals for families.
• Participate in collaborative case planning and review meetings.
• Deliver case management activities including supervised visitation, parent aide support, behavioral aide, transportation, crisis intervention, and advocacy.
• Complete in-home visits to monitor progress and provide ongoing support for families involved with DFCS.
• Maintain documentation of services rendered and ensure alignment with billing and reporting standards.

Education

Bachelor’s degree in a human services-related field

High School Diploma/GED with 5+ years verifiable field experience
