Qualifications

Responsibilities

• Conduct initial client intakes and assess referred women and infants needs.
• Explain home visiting options and refer to appropriate providers.
• Provide information on breastfeeding, health care, parenting education.
• Enter participant data accurately into the Well Family System and maintain confidentiality.
• Travel to contact clients in the service area as required.
• Coordinate with Healthy Start staff and community partners.
