Job summary

Florida Department of Health in Pinellas County seeks a HUMAN SERVICES COUNSELOR I to establish trust with families, conduct home visits, and provide case management and parent education under Healthy Families Florida standards. This role involves coordinating with a multidisciplinary team and maintaining documentation in HMS for up to 5 years of service.

Responsibilities include linking families to community supports, tracking prenatal and infant health care, and ensuring timely service

Qualifications

• At least one year of experience working with at risk children and/or families.
• Valid driver’s license with daily access to a reliable personal vehicle.
• Ability to work a flexible schedule to include evenings and weekends, as needed.
• High school diploma or its equivalent, as mandated by the policies of Healthy Families Florida.
• Bilingual with written and spoken fluency in English and Spanish (preferred).

Responsibilities

• Establishes a trusting relationship with families and maintains regular client contact.
• Provides case management, care coordination, and parent education through home visits up to 5 years.
• Serves as primary coordinator on a multidisciplinary team and maintains case weight per program requirements.
• Assists families with accessing community supports and services.
• Documents case management activities and maintains electronic health records in HMS.
• Meets grant goals, performance measures, and program/funder requirements.
• Attends supervision and regular staff meetings to discuss case findings and activities.
• Performs all other duties and completes reports as assigned.
