Job summary

Sacramento Children’s Home is seeking a Home Visitor to provide home and community-based support to families with children 0–17 in Sacramento, CA. You will conduct strengths and needs analyses, develop family nurturing plans, and link families to community resources.

Position involves 65% home visitation, 20% documentation, and 15% meetings. Schedule is Mon–Fri, 8:30 am–5:00 pm, with potential after-hours as needed. Salary starts at $21.97–$24.70 per hour.

Qualifications

• Must be able to read, write, speak and understand English.
• Ability to establish and maintain appropriate boundaries with families.
• Knowledge of child development and behavior.

Responsibilities

• Lead 65% home visitation with families to assess needs and implement nurturing plans.
• Coordinate with multi-resource team and advocate for families’ access to services.
• Prepare case notes, assessments, and Family Nurturing Plans; document in files.
• Attend meetings and provide additional support to Family Resource Center.
