Job summary

Sacramento Children's Home is seeking a Home Visitor to provide home and community-based support to at-risk families. You will work with a multi-resource team to assess family strengths, risks, and needs, and to develop individualized nurturing plans for children aged 0-17.

Role requires a high school diploma or GED; an AA degree is preferred. Bilingual Spanish is preferred. Driving is required; a personal vehicle and valid license are necessary.

Qualifications

• High school diploma or GED required.
• AA degree in Early Childhood Education, Human Development or a similar field preferred.
• Spanish language proficiency preferred.
• Driving is an essential function of this job.

Responsibilities

• Provide home visitation services to families with children 0-17.
• Develop and implement Family Nurturing Plans with the team.
• Maintain accurate case files and progress notes.
• Complete required reports and documentation on time.
• Coordinate with community resources to support families.
