Job summary

Sacramento Children's Home in Sacramento, CA, is seeking a Home Visitor to work with a multi-resource team delivering home and community-based support to families identified as at risk for child abuse and neglect.

Responsibilities include conducting strengths and needs analyses, developing family nurturing plans, providing home visitation for ages 0-17, connecting families with community resources, and maintaining accurate case files and progress notes; standard hours are Monday–Friday, 8:30

Qualifications

• High school diploma or GED required.
• AA degree in Early Childhood Education, Human Development or similar preferred.
• Knowledge of child abuse issues and child development recommended.

Responsibilities

• Conduct in-depth strengths, risks, and needs analysis for each family.
• Develop and implement a Family Nurturing Plan with the team.
• Provide home visitation services for children aged 0–17.
• Assist families with problem-solving and coping skills.
• Promote parenting skills and positive child guidance.
• Document all communications and maintain case files and progress notes.
• Coordinate with community resources and health/school partners.
• Attend staff and multi-resource team meetings as required.
