Job summary

Sacramento Children's Home is seeking a Home Visitor to work full‑time in North Sacramento, CA. You will conduct family assessments, develop nurturing plans, and provide in‑home support to at‑risk families with children 0–17.

You’ll collaborate with a multi‑resource team, document progress, and connect families to community services while promoting healthy parenting practices. Valid driver’s license required and 21+ years old.

Qualifications

• High school diploma or GED required.
• AA degree in Early Childhood Education or related field preferred.
• Spanish proficiency preferred.
• Valid driver’s license and access to personal vehicle required.
• Must be at least 21 years old.

Responsibilities

• Conduct in-depth strengths, risks, and needs analysis of all family members referred to the program.
• Develop and implement a Family Nurturing Plan in collaboration with the team.
• Provide home visitation services to families with children aged 0–17.
• Assist families in problem‑solving and coping skills.
• Promote positive parenting skills and age-appropriate activities for children.
• Document progress notes and maintain case files.
• Utilize community resources to meet family goals and advocate for families.
