Job summary

Sacramento Children's Home in Sacramento, CA seeks a Home Visitor to support families at risk. You will work with a multi-resource team to deliver home and community-based services to children aged 0-17 and their families, following program guidelines and safety policies.

The role emphasizes strengthening parenting skills, conducting assessments, developing Family Nurturing Plans, and coordinating referrals to community resources; a valid CA driver’s license is required.

Qualifications

• High school diploma or GED required.
• AA degree in Early Childhood Education or Human Development preferred.
• Driver’s license and own vehicle required.
• Must be at least 21 years old.
• Bilingual fluency may be required based on client needs.

Responsibilities

• Conduct strengths, risks, and needs analysis for all family members.
• Develop and implement a Family Nurturing Plan with each family.
• Provide home visitation to children aged 0–17.
• Promote parenting skills and positive discipline.
• Record communications and progress notes in case files.
• Coordinate with multi-resource team and community resources.
