Job summary

WellSpace Health in North Highlands, CA is seeking a Home Visitor to deliver parenting education and connect families with community resources in homes or community settings. The role emphasizes fidelity to the program model, client documentation, and regular trainings.

The Home Visitor will conduct outreach, facilitate Family Resource Center events, and coordinate service plans for families with children ages 0-17, with occasional evenings and weekends.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s degree in Social Work, Child Development or Psychology preferred.
• Two+ years of related client experience, or an equivalent combination of education and experience.
• Bilingual fluency in English with additional languages preferred: Farsi/Dari/Pashto/Russian/Spanish.
• Strong administrative, time management and organizational skills.

Responsibilities

• Conduct regular home and community visits to assigned families and children.
• Document notes from visits and maintain client charts and program records.
• Provide health information and facilitate parent-child activities per program model.
• Plan and lead Family Resource Center groups and classes.
• Travel between office, homes and community sites using own vehicle.
