It uses Triple P and other tools to screen, liaise with mental health resources, and coordinate care across the community. Expect ~75% direct family services and ~25% for documentation, data collection, and reporting.

Qualifications

• High School Diploma or Equivalent Required - And Associate's Degree Or Bachelor's Degree Preferred
• Two Years Healthcare - Ambulatory Care Required

Responsibilities

• Provide direct services to families ~75% of each work week.
• Screen and refer to mental health resources as needed; support engagement and care coordination.
• Support documentation, data collection and reporting activities (~25% of time).
• Assist with training and networking with community resource representatives as needed.
