Job summary

ConnectFamilias, Inc. is seeking a Family Success Coach to deliver strength-based screenings, develop individualized case plans, and coordinate wraparound services for children and families in Miami‑Dade County.

This on-site, non-clinical role emphasizes engagement, coaching, and collaboration with community providers; bilingual English/Spanish is preferred. Two years of relevant experience and a related degree are required, with CANS certification training provided.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related field is required.
• CANS certification must be obtained within 20 days of employment (training provided).
• Minimum 2 years of experience in related social services, managing caseload, and providing prevention/early intervention services.

Responsibilities

• Conduct strength-based assessments to identify family needs and develop individualized success plans.
• Connect families to services and resources through care coordination.
• Facilitate one-on-one and small group coaching sessions to support family goals.
• Collaborate with community providers to ensure seamless service coordination.
• Maintain accurate electronic documentation of assessments, case plans, and coaching session notes.
