Job summary

You will conduct intake, provide case management, coordinate referrals, and report progress to leadership.

Qualifications

• High school diploma required.
• 2–3 years’ experience working with adults/youth programs.
• Experience in community outreach.
• Valid driver’s license and reliable transportation.
• Background/drug test screening required.

Responsibilities

• Assist with intake and assessment for new families annually.
• Provide case management and facilitate referrals for community resources.
• Counsel individuals and families by delineating alternatives and goals.
• Report progress to Director of Family Services.
• Assist in grant reporting requirements.
• Maintain 50% family engagement among OYC families.
• Conduct monthly parent/community meetings.
• Serve as advisor and success coach for educational/employment goals.
• Prepare participants for college or career transitions.
• Coordinate with program staff to achieve recruitment and retention goals.
• Develop and implement communication strategies for opportunities.
• Monitor participant progress and maintain case notes.
