Job summary

Sacramento Children's Home in Sacramento, CA is seeking a Family Partner to support families within the Wraparound program. You will advocate for caregiver voice, coordinate with the Child and Family Team, and connect families with needed services. This full-time role requires strong communication, cultural competence, and the ability to navigate housing, education, and child welfare systems.

Must have a high school diploma or GED; a BA is preferred. Driving and a CA license are required.

Qualifications

• High school diploma or GED required; BA preferred.
• Previous mental health or social service experience preferred.
• Must have experience as a parent or caregiver of a child with serious emotional disturbances.
• CPR/First Aid certification preferred or obtained within agency-required timeframe.
• Valid CA driver’s license and minimum insurance coverage required.

Responsibilities

• Provide one-on-one support and peer counseling to caregivers.
• Orient caregivers to the CFT process and High Fidelity Wraparound (HFW) service.
• Participate in treatment planning and case conferences as appropriate.
• Support families with education rights during special education processes.
• Develop housing plans and connect families to housing resources.
• Document all services to families/clients per agency and county requirements.
• Attend meetings and trainings to advocate for family needs.
• Carry out mandated reporter duties as required.
• Establish and maintain collaborative relationships with staff and external resources.
