National Youth Advocate Program (NYAP) is seeking a Family Service Coordinator in South Bend, IN to support youth and families with in-home services, advocacy, and coordinated community resources. The role emphasizes safety, stability, and successful community living.

The position requires a bachelor’s in social work or related field, flexible hours, and a valid driver’s license. NYAP offers competitive benefits, mileage reimbursement, and opportunities for professional growth.

Formación

• Bachelor's Degree in Social Work or comparable Human Services field from an accredited institution.
• 2 years of work experience in direct service with youth and families strongly preferred.
• Must have flexible availability and a valid driver's license with reliable transportation.
• Must meet car insurance requirements of 100,000/300,000 bodily injury.
• Willingness to work flexible and non-traditional hours in service of families.

Responsabilidades

• Actively supports and promotes the organization's mission, vision, and values.
• Provides in-home supportive services, transportation, parenting education, counseling, and community referrals.
• Travels daily to deliver community-based services to youth and families.
• Completes required documentation and trainings in a timely manner.
• Coordinates and monitors services across mental health, medical, education, and social services.
• Reviews and updates youth-based plans to ensure progress and outcomes.
