Job summary

ELADC is seeking a Family Service Coordinator to support children and families within the Early Childhood program. This non-exempt position focuses on coordinating services and IFSP development, under the supervision of the Early Childhood Director.

The role involves intake interviews, family outreach, development of IFSPs, service coordination, and ongoing documentation, with duties following program guidelines and safety policies.

Qualifications

  • Bachelor's degree required in a relevant field.
  • Experience coordinating services for children and families preferred.

Responsibilities

  • Interpret and implement department goals with the Early Childhood Director.
  • Promote safety and enforce health policies.
  • Lead placement process for children referred by the Early Childhood Director.
  • Coordinate intake interviews and family communications.
  • Develop IFSP with family and monitor progress.

FAMILY SERVICE COORDINATOR

Position Title: Family Service Coordination

Status: Non-Exempt

Minimum Qualifications: Bachelor's degree, emphasis in one of the following areas: Child Development, Early Childhood Education, Early Childhood Special Education or Special Education or registered nurse with two years' experience in pediatrics or child development.

Maximum Qualifications: MA Certification, emphasis in one of the following areas: Child Development, Early Childhood Education, Early Childhood Special Education or Special Education.

Supervised By: Early Childhood Director

Supervises: Developmental Specialist I, Ancillary Staff

Working Hours: Determined by Early Childhood Director

Duties and Responsibilities:

1. Interpret and implement department goals, objectives and policies ensuring delivery of quality services in collaboration with the Early Childhood Director.

2. Promotes safety, ensuring that health and safety policies and procedures are followed.

3. Acts as team leader in the placement process of children referred by Early Childhood Director.:

  • Carry out intake interview with family;
  • Disseminate FIT information to families;
  • Provide interim service coordination activities to families;
  • Facilitate the scheduling of the evaluation team;
  • Facilitate the evaluation teams’ evaluation results;
  • Review with family the evaluation teams’ evaluation results;
  • Develop IFSP with family.

4. Administer and coordinate developmental re-assessment of children every six months.

5. Provide direct developmental services to children and families as designated on the IFSP.

6. Provide service coordination to children and families when identified as service coordinator.

7. Monitor and document the implementation and success of the outcomes and activities identified on the IFSP on service coordination designated children.

8. Conduct IFSP team review on assigned children at least every six months to determine progress, continued eligibility, programming, and possible need for reassessments.

9. Supervise and monitor the implementation and documentation of therapy services and their outcomes on service coordination designated children.

10. When appropriate coordinate with playgroup staff to ensure that activities are followed through in accordance with children’s individual outcomes on IFSP.

11. Provide a smooth and effective transition to 3-year-old programs, following the transition process outlined by DOH-FIT program guidelines and the memorandum of understandings established with receiving agencies.

12. Maintain children’s records meeting the Long-Term Services Division, Children Youth and Families Department, accrediting agencies’ and ELADC’s standards and requirements.

13. Participate in the planning and reporting of the organization’s Outcome Management System.

14. Develop, implement and or attend meetings, trainings and conferences as directed by the Early Childhood Director.

15. Provide accurate and timely billing information as requested by the Early Childhood Director.

16. Document general progress on service coordination designated children on a monthly basis.

17. Follow agency’s personnel policies and procedures.

18. Other duties as assigned.

Physical, Mental, and Visual Skills:

  • Good organizational skills.
  • Ability to use logical thinking to solve problems.
  • Ability to perform light to medium work lifting and/or carrying of objects weighing up to 30 pounds, performing such activities as stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, and grasping.
  • Use personal vehicle and/or agency vehicle to make home visits or to conduct other business as needed. Must have a clear driving record to use agency vehicle. Mileage is rendered for use of own personal vehicle.

Working Environments: The major activities of the job are carried out in the following: Indoors 65%, outdoors 20%, and 15% in a car or van.
