Qualifications

Responsibilities

• Manage referrals and schedule developmental evaluations to determine eligibility for Part C.
• Serve as a point of contact to help families obtain services, funds, and assistance.
• Coordinate evaluations and identify service providers for the child and family.
• Maintain documentation and ensure compliance with policies and timelines.
• Collaborate with insurers, school districts, and other agencies as needed.
• Support IFSP development and monitor family-centered goals.
• Maintain confidentiality of client information.
