Job summary

LifeLong Medical Care seeks a Family NP/PA to provide outpatient primary care at the Downtown Oakland Health Center in Oakland, CA. Qualifications

• Master’s or Doctorate degree from an accredited Nurse Practitioner program.
• Current California RN license and NP license.
• National NP Certification in Family, Adult-Gerontology, or related field.

Responsibilities

• Perform physical examinations, obtain histories, diagnose illnesses, develop treatment plans, and prescribe therapies.
• Provide preventive and primary care services including immunizations and screenings.
• Manage chronic conditions with emphasis on patient education and self-management.
