Job summary

OLE Health seeks a Family Nurse Practitioner to provide high-quality primary care across multiple Napa-Solano campuses. You will work in a collaborative team, manage diverse patient needs, and contribute to a patient-centered, inclusive care model.

Qualifications

Master of Science in Nursing required
California FNP certification required
Must have or obtain California medication furnishing license
Must have or obtain DEA License
CPR Certification required
Experience in a community healthcare center preferred
Bilingual preferred
Strong time management skills in a fast-paced environment
Knowledge of Electronic Health Records systems

Responsibilities

Provide health care services: assessment, diagnosis, treatment, planning and education
Manage acute care, periodic re-evaluations, and chronic conditions
Review labs, imaging, prescriptions, and referrals
Prescribe and manage medications; ensure safety and effectiveness
Refer patients to specialists and coordinate care
Document in EHR and communicate with patients and staff
Coordinate patient care with nursing and support teams
Participate in quality improvement and outreach programs
