Job summary

Amplify Health is seeking a Family Nurse Practitioner to deliver comprehensive, patient-centered primary care across the lifespan in a community health center setting.

The NP will diagnose, treat, and manage acute and chronic conditions, provide preventive care, and promote health equity for underserved populations in alignment with the organization’s mission. Strong communication and collaboration with a multidisciplinary team are essential.

Qualifications

• Nurse Practitioner degree from an accredited institution.
• Current North Carolina NP license.
• Completed residency training (REQUIRED).
• At least one year of experience in a primary care setting functioning at the full extent of training and licensure.
• Basic Life Support within 30 days of hire or transfer.
• Experience with EPIC Electronic Medical Records system preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct thorough patient assessments, including medical history, physical examination, and diagnostic tests.
• Diagnose and treat various medical conditions and illnesses.
• Prescribe medications and monitor their effectiveness.
• Develop treatment plans.
• Collaborating with other healthcare professionals to ensure the best possible patient care.
• Maintaining accurate and up-to-date medical records.
• Educate patients on disease prevention, medication adherence, and lifestyle modifications.
• Coordinate care with physicians, nurses, behavioral health providers, care managers, and specialists.
• Participate in team huddles, case conferences, and clinical meetings.
