LifeLong Medical Care seeks a Family Nurse Practitioner for our Downtown Oakland Health Center to deliver comprehensive outpatient primary care in a collaborative, team-based setting. This role focuses on serving underserved communities, managing chronic conditions, and integrating behavioral health services within the care model.

Qualifications

• Master’s or Doctorate degree from an accredited NP program.
• Current California RN and NP licenses.
• National certification as an NP in Family, Adult-gerontology, or relevant specialty.
• Furnishing license and DEA certification required or obtainable upon hire.
• Minimum 1–2 years of clinical experience preferred (new grads may be considered).

Responsabilités

• Perform physical examinations, obtain medical histories, diagnose illnesses, develop treatment plans, and prescribe medications.
• Provide preventive and primary care services, including immunizations, health screenings, and wellness counseling.
• Manage chronic conditions such as diabetes, hypertension, asthma, and depression with patient education.
• Collaborate with an interdisciplinary care team to address medical, behavioral, and social determinants of health.
• Provide acute care visits and triage support; support referrals and follow-up for diagnostics and specialty care.
• Participate in clinical huddles, staff meetings, and quality improvement initiatives.
• Serve as a preceptor for students or trainees when appropriate.
