Job summary

Health Solutions is expanding and hiring a full-time Family Nurse Practitioner for the Montebello Family Medicine location. You will provide comprehensive primary care across all ages, perform exams, order tests, and manage acute and chronic conditions in a patient-centered, team-based environment aligned with PCMH principles.

You will collaborate with physicians, behavioral health clinicians, care coordinators, and staff to ensure quality care, preventive services, and coordinated transitions

Qualifications

• Licensed as Advanced Practice Nurse in CO or a compact state.
• Current D.E.A. license.
• Current CPR, ACLS or Advanced Life Support within 3–12 months of employment.

Responsibilities

• Deliver comprehensive primary care across the lifespan.
• Order, perform, and interpret diagnostic tests.
• Develop and adjust treatment plans with prescribed meds and preventive care.
• Deliver preventive services and vaccinations.
• Collaborate with multidisciplinary team to ensure continuity of care.
• Maintain accurate electronic health records and compliant documentation.
