Job summary

Health Solutions is seeking a Family Nurse Practitioner to join their Medical Services Team in Pueblo, CO. You will provide comprehensive primary care for adults and children, coordinate care, and participate in quality initiatives within a PCMH framework.

Under supervision, you will work with physicians, nurses, and care coordinators to deliver patient-centered care, manage chronic conditions, and improve health outcomes while adhering to state regulations and organizational policies.

Qualifications

• Licensed as an Advanced Practice Nurse in CO or compact state.
• Current DEA license.
• CPR and ACLS certifications.
• Minimum of 5 years’ experience as an RN.

Responsibilities

• Provide comprehensive primary care across the lifespan using evidence-based guidelines.
• Order, perform, and interpret diagnostic tests to support accurate diagnoses.
• Develop and adjust treatment plans, prescribe medications and preventive interventions.
• Deliver preventive services, wellness visits, vaccinations, and routine check-ups.
• Collaborate with a multidisciplinary team to ensure continuity of care.
