Job summary

Health Solutions is expanding and hiring a full-time Primary Care Family Nurse Practitioner to join our Health Solutions Family Medicine team in Pueblo, CO. The role offers a competitive salary and a $10,000 sign-on bonus, with comprehensive benefits and strong team support.

You will provide full-scope primary care to adults and children, perform tests, develop treatment plans, and collaborate within a multidisciplinary group to ensure high-quality, patient-centered care.

Qualifications

• Licensed as Advanced Practice Nurse in Colorado or a compact state.
• Current DEA license.
• Current CPR and ACLS certification (within 3–12 months if applicable).
• Minimum 5 years’ RN experience and 3 years’ Family Nurse Practitioner experience.
• Insurable under company-provided malpractice coverage.
• Computer proficiency for electronic health records and documentation.
• Patient-centered service: kindness, compassion, professionalism.
• Clear communication and patient education throughout care.

Responsibilities

• Provide comprehensive primary care across the lifespan with evidence-based guidelines.
• Order, perform, and interpret diagnostic tests to support diagnoses.
• Develop and adjust treatment plans, prescribe medications and preventive care.
• Deliver preventive services and wellness visits, vaccinations, screenings.
• Engage patients in shared decision‑making and self-management.
• Review labs and coordinate care with the team; manage medications accordingly.
• Coordinate referrals and transitions across providers and settings.
• Maintain accurate EHRs; ensure compliant billing and quality metrics.
