Job summary

Health Solutions is hiring a full-time Primary Care Family Nurse Practitioner to join our Family Medicine/MARC team in Southern Colorado. The FNP will provide adult and pediatric primary care, including exams, diagnostics, treatments, and preventive services, while collaborating with physicians, nurses, and behavioral health staff.

You will deliver patient-centered care aligned with PCMH principles, participate in care planning, and ensure compliant documentation and billing.

Qualifications

• Licensed Advanced Practice Nurse in CO or compact state.
• Current DEA registration.
• CPR, ACLS or Advanced Life Support current or within 3–12 months.
• Minimum 5 years’ RN experience; malpractice coverage.
• Computer proficiency and electronic health records experience.
• Spanish language skills desired.

Responsibilities

• Provide comprehensive primary care across lifespan.
• Order, perform, and interpret diagnostic tests.
• Develop and adjust treatment plans; prescribe medications.
• Deliver preventive services and screenings.
• Collaborate with a multidisciplinary team to coordinate care.
• Maintain accurate EHR documentation and meet quality metrics.
• Participate in huddles, care planning, and referrals.
