Job summary

Grace Community Care & Homes is seeking a Family Nurse Practitioner to deliver assessments, preventive care and chronic-condition management within our Medical and Preventive Care department, collaborating with the medical director and clinical team.

This is a part-time, on-site role in New Jersey with schedule discussed during hiring; active NJ RN and APN credentials, prescribing authority and outpatient experience are required; pay is $60-$70 per hour; benefits include paid time off.

Job description

Grace Community Care & Homes is developing its Medical and Preventive Care department and seeks a Family Nurse Practitioner. The role will contribute to person-centered services, reliable operations and accurate documentation.

Core responsibilities

Provide assessments, preventive care, chronic-condition management, treatment and referrals; document care and collaborate with the medical director and clinical team.

Work collaboratively with department leadership, intake, credentialing, billing and quality teams. Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope. Participate in orientation, competency review and quality improvement.

Program readiness

This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope.

Location and schedule

New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. Part-time; assignment and weekly schedule discussed during hiring. Clinical services begin only at approved locations and within authorized scope.

Active NJ RN and APN credentials and appropriate family certification; required prescribing authority and practice arrangements; outpatient experience. Applicants should provide relevant credentials, experience, availability and a resume. Final regulatory eligibility will be verified before appointment or service delivery.
