Job summary

Grace Community Care & Homes is expanding its Medical and Preventive Care division and seeks a Family Nurse Practitioner to provide assessments, preventive care and chronic-condition management in outpatient settings. This part-time role is based in New Jersey with on-site or community assignments as applicable.

Start date and duties are contingent on approvals and staffing readiness. Compensation ranges $60-$70 per hour, with paid time off; assignment and schedule discussed during hiring.

Qualifications

• Active New Jersey RN license and APN credentials.
• Family Nurse Practitioner certification.
• Prescribing authority and outpatient practice experience.

Responsibilities

• Provide assessments, preventive care, chronic-condition management, treatment and referrals.
• Document care and collaborate with the medical director and clinical team.
• Work with department leadership, intake, credentialing, billing and quality teams; maintain confidentiality and respect patient choices; communicate service gaps promptly and follow scope.
• Participate in orientation, competency review and quality improvement.
