Job summary

Grace Community Care & Homes is seeking a Family Nurse Practitioner to deliver assessments, preventive care and chronic-condition management within our Medical and Preventive Care department, collaborating with the medical director and clinical team.

This is a part-time, on-site role in New Jersey with schedule discussed during hiring; active NJ RN and APN credentials, prescribing authority and outpatient experience are required; pay is $60-$70 per hour; benefits include paid time off.

Qualifications

• Active NJ RN and APN credentials required.
• Prescribing authority and appropriate family certification.
• Outpatient experience preferred.

Responsibilities

• Provide assessments, preventive care and chronic-condition management; document care and referrals.
• Collaborate with medical director and clinical team; coordinate with leadership, intake, credentialing, billing and quality teams.
• Maintain confidentiality, follow scope and policies; participate in orientation and quality improvement.
