Job summary

Advocates is seeking a Person-Centered Services Navigator to provide navigation, service coordination, and individualized planning support for DDS-eligible individuals with autism and developmental disabilities. Travel across Eastern Massachusetts is required, with a schedule Monday–Friday 9am–5pm, and occasional evenings or weekends as needed by families.

The Navigator supports families in accessing services, building skills, independence, and community participation, including DESE/DDS

Qualifications

• Bachelor’s degree in human services, social work, psychology, education, special education, or a related field preferred.
• Experience working with individuals with autism, developmental disabilities, and/or complex family support needs.
• Knowledge of DDS services, DESE/DDS programming, Agency with Choice, family navigation, self-direction, IEPs, community-based support, and service coordination preferred.

Responsibilities

• Provide person-centered navigation and supports aligned with DDS and agency requirements.
• Identify strengths, needs, priorities, goals, and barriers for service access and independence.
• Develop, implement, and monitor individualized service plans, goals, budgets, and strategies with stakeholders.
