Job summary

Acloché LLC is seeking a Physician specializing in Family or Internal Medicine to join a full-time, Monday–Friday practice in Valparaiso, IN. The position allows a flexible schedule and a 1:10 call rotation within a leading, tech-forward healthcare environment.

The role focuses on high-quality primary care, chronic disease management, and comprehensive preventive services for adults (and all ages for FM). Relocation assistance is available.

Qualifications

• Board Certified or Board Eligible with a plan for certification within a specified timeframe.
• Possession of Indiana Medical License and CSR/DEA registration
• Experience with EPIC EMR and AI-assisted documentation tools
• Strong communication skills and EMR proficiency

Responsibilities

• Provide high-quality primary care, including exams, wellness visits, and immunizations.
• Clinic patients per day: 15-30.
• Call Rotation: 1:10.
• Diagnose and manage chronic conditions (e.g., HTN, DM, asthma, COPD).
• Treat non-emergent injuries and illnesses; infections and minor trauma.
• Order and interpret labs, X-rays, and other diagnostics.
• Coordinate patient care with specialists.
