Job summary

You will work with families to set achievable goals, provide education on nutrition and fitness, and documentation in the patient record. Remote work one day weekly may occur after training.

Qualifications

• Requires High School diploma or GED.
• Associate degree preferred in Human Services field.
• 1-3 years related work experience required.
• Excellent verbal and written communication skills.
• Experience with computer systems including MS Office applications.

Responsibilities

• Determines resources to meet patients and families' needs.
• Develops service plans for complex cases.
• Acts as case manager to coordinate care with providers.
• Makes decisions about services to recommend to families.
• Maintains a caseload with multiple daily contacts and documentation.
