Job summary

Beth Israel Lahey Health is seeking a dedicated Case Manager for the Children's Hospital Healthy in the City program in Dorchester, MA. You will assess resources, coordinate with social work and medical teams, and help families access services to support healthy, active lives.

You will conduct home visits as needed, build community liaisons, and provide ongoing training related to resources to staff and families. Strong communication and advocacy are essential in this patient-centered role.

Qualifications

• Requires High School diploma or GED; Associate's degree in Human Services preferred.
• 1-3 years related work experience required; minimum 2 years without degree.
• Excellent verbal and written communication skills.
• Experience with computer systems including web-based applications and MS Office.

Responsibilities

• Determines resources to meet patients' and families' needs, independently or with staff.
• Develops a multifaceted service plan to meet complex needs.
• Acts as case manager to coordinate care with medical providers.
• Maintains a caseload with multiple patient contacts daily (phone, paperwork, in-person).
• Documents timely in the online medical record and participates in team meetings for coordinated care.
• Advocates for patients in written and verbal communications and connects to community resources.
