Responsibilities

• Collaborate with family and CPS to ensure effective service implementation and adjustments as needed.
• Develop and monitor service plans, refer to community resources, and document progress and outcomes.
• Coordinate initial family team meetings and update Crisis Management Plan as needed.
• Complete ISP reviews and maintain program documentation in line with requirements.
