Job summary

Canopy Children's Solutions is seeking a dedicated Case Manager for the in-Circle Program in Tupelo, MS. You will assess needs and connect children and families with social, educational, vocational, and community services, coordinating with CPS for effective service delivery.

Collaborate to develop service plans, monitor progress, and document outcomes. A social work background and strong communication skills are essential to support families and drive positive change.

Qualifications

• Bachelor’s degree in Social Work or related field.
• At least two (2) years’ experience with children and families.
• Competence with children and families in the child welfare system.
• Proficient in completing documentation and reports.
• Must be at least 21 years old with a valid license and auto liability insurance; willing to use personal vehicle.

Responsibilities

• Assess service needs and link child/family with wrap-around services.
• Act as an adult role model and collaborate with families to create supportive environments.
• Work with CPS to implement services and adjust as needs change.
• Perform comprehensive case management including assessment, service planning, referral, and monitoring.
• Coordinate with families to ensure progress toward service goals and proper documentation.
• Conduct family team meetings and update Crisis Management Plan as needed.
• Provide direct case intervention in home, school, office, and community settings.
