Job summary

Qualifications

• Bachelor’s degree in Social Work or a related field with at least two years’ experience working with children and families.
• Demonstrate competence in working with children and families in the child welfare system.
• Demonstrates competence in completing documentation and reports related to services provided.
• Computer skills required; must be willing to use personal vehicle for organization business.

Responsibilities

• Coordinate with local MCPS staff to contact families within 24 hours of referral acceptance.
• Assist eligible individuals to access medical, social, educational, vocational and other services.
• Ensure case management services are documented in compliance with program requirements.
• Monitor progress and document outcomes; maintain records for each family served.
• Develop initial case management goals and interventions with the HopePath Solution therapist, MCPS, and the family.
• Facilitate initial family team meetings and update crisis plans as needed.
• Complete ISP reviews and monitor progress toward service goals.
• Assess strengths and needs of families and connect them with community resources.
• Serve as a positive role model in all interactions with participants.
• Provide direct case intervention services in home, school, office, and community settings.
