Job summary

Canopy Children's Solutions in Mississippi is seeking a Case Manager for the in-Circle Program to assess service needs and link children and families to wrap-around services tailored to individualized plans.

You will collaborate with CPS caseworkers, develop specific service plans, coordinate referrals, monitor progress, and document outcomes to support families across home, school, and community settings.

Qualifications

• Bachelor’s degree required in Social Work or a related field and at least two years’ experience with children and families.
• Demonstrates competence in working with children and families in the child welfare system.
• Demonstrates competence in completing documentation and reports related to services provided.
• Computer skills required.
• Must be at least 21 years of age, have a valid driver license, good driving record, proof of auto liability insurance, and willing to use personal vehicle for organization business.

Responsibilities

• Coordinate with local MCPS staff and contact the family within 24 hours after referral.
• Assist eligible individuals to gain access to medical, social, educational, vocational and other services.
• Ensure case management services are provided and documented within programmatic requirements.
• Monitor participants’ progress and document accordingly.
• Complete initial case management goals, objectives and interventions for the case plan with the in-Circle Program therapist, MCPS and the family.
• Facilitate initial family team meetings and subsequent reviews per the Individualized Service Plan.
