Job summary

Cornerstone Community Services seeks a Family Resource Specialist to advance Healthy Families Indiana through community outreach, referrals, family engagement, and assessments. You will build partnerships with hospitals, DCS, WIC, and schools to connect families with services and resources.

The role requires strong communication, relationship-building, and organizational skills to support families and leave a positive community impact.

Qualifications

• High school diploma or equivalent required.
• Experience working with or providing services to children and families.
• Strong interpersonal and written communication skills.
• Ability to establish trusting relationships with families.
• Knowledge of infant and child development is a plus.

Responsibilities

• Develop and maintain referral sources to support program enrollment and service capacity.
• Engage referred families with family-centered outreach and assessments.
• Explain program eligibility, services, and assessment processes to families.
• Complete FROG Scale with eligible families following required protocols.
• Maintain accurate documentation and data in designated systems.
• Collaborate with community partners to connect families with resources.
