Job summary

Community Teamwork in Lowell, MA is seeking an Interim Family Service Worker to provide entry-level case management to families, support engagement, and promote school readiness within CTI's Early Learning Program. The role requires coordinating with families, conducting intake, developing partnership agreements, and collaborating with classroom staff to address barriers and achieve measurable outcomes.

Qualifications

• Bachelor's degree in Human Services preferred.
• 3-5 years of experience with low-income individuals or families.
• Desire to work with diverse, at-risk populations.
• Excellent customer service, organizational, oral and written communication skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Proficient with basic computer software and data collection systems.
• Proof of MMR and recent physical.
• CORI/DCF background check and fingerprinting.
• Valid Driver's License.

Responsibilities

• Welcome families and encourage engagement in program decisions.
• Complete intake and needs assessments with each family.
• Maintain program rules, policies, and case management visits.
• Home visits for CTI's Early Learning Program at least semi-annually.
• Develop Individualized Family Partnership Agreements.
• Establish, evaluate and monitor goals with measurable outcomes.
• Advocate for clients and coordinate services with interdisciplinary teams.
• Provide coverage at sites/classrooms as needed.
• Integrate children with disabilities into CTI programs and train staff as needed.
• Interact with professional agencies to ensure proper implementation of recommendations.
• Plan and facilitate Parent Committee meetings.
• Recruit families to participate in the program.
• Monitor attendance and maintain proper documentation.
• Maintain accurate case records and data entry.
• Provide emergency response management and uphold CTI values.
