Community Teamwork in Lowell, MA is seeking a Case Manager to help clients build self-sufficiency plans, advocate on their behalf, and attend administrative hearings when needed. The role may involve home visits and unit inspections to assess progress and needs.

You will maintain accurate case records, enter data into agency databases, and support emergencies, while upholding CTI values of Equity, Integrity and Compassion.

Qualifications

• High School Diploma or Equivalent required.
• Bilingual Spanish required.
• Knowledge and experience with low income, culturally diverse, and at-risk families and individuals.
• Excellent customer service, organizational, oral and written communication skills.
• Ability to manage multiple priorities and work in a fast-paced environment.
• Proficient computer skills in basic software and data collection systems.
• Excellent crisis management skills.

Responsabilités

• Help clients build self-sufficiency plans and advocate on their behalf.
• Attend administrative hearings if necessary.
• May perform home visits and/or unit inspections.
• Maintain structure and policies through orientation of participants by reviewing rules and visits.
• Maintain accurate case records by documenting services and referrals; enter data into databases.
• Provide emergency response management to address crises or safety issues.
• Uphold the values of CTI - Equity, Integrity, Compassion.
