Qualifications

• Bachelor's Degree required in social science or related field.
• Minimum of two years post-secondary education or equivalent in human services.
• Minimum of two years direct service or case management in related programs.
• Valid driver's license, driving record and reliable transportation.
• Clearances: PA State Police, PA Child Abuse History and FBI.
• Must align with the agency's Mission Statement and Core Values.

Responsibilities

• Adapt hours including evenings, weekends and on-call on a rotating basis.
• Travel independently to clients' homes for service delivery.
• Empower families to gain independence through skill building and routines.
• Implement the Family Empowerment Model with intensive initial engagement.
• Hold families accountable to handbook requirements and progress goals.
• Make day-to-day decisions and inform leadership of high-risk cases.
• Engage in goal-oriented, trauma-informed interactions with clients.
• Provide on-call services and develop crisis plans for consumers.
• Maintain fidelity to Family Empowerment service models.
• Communicate regularly with leadership about operations and needs.
• Deliver services aligned with Glade Run’s mission of wellness and growth.
• Model trauma-informed, culturally responsive practices and collaborate with team.
• Participate in team meetings and share insights on outcomes.
