Job summary

Community Services Group in Lancaster, PA is seeking a Case Manager to provide casework services to help individuals and families achieve their best functioning. You will exercise clinical judgment and coordinate services with families and schools, reporting to the Program Director.

The role focuses on Children's Blended Case Management services, with Monday–Friday hours 8:30am–5:00pm and on-call rotation. Starting wage is $20.50/hour, negotiable based on experience.

Qualifications

• Bachelor's degree in sociology, social work, psychology, gerontology, anthropology, criminal justice, theology, nursing, political science, history, counseling, or education.
• RN or HS diploma with specified credits and experience in human services with direct client contact, or 5 years of mental illness direct care in human services.

Responsibilities

• Interviews individuals and family members to gather demographic and psychosocial information.
• Completes strength-based assessments and need analyses with individuals and families.
• Develops goal-oriented, time-limited service plans with the individual and/or representatives.
• Coordinates services for individuals and families with relevant providers.
• Maintains accurate records and produces clear reports.
• Participates in training to understand company programs, services, laws, and procedures.
• Makes referrals to public/private social services and helps families utilize resources.
• Maintains communication with families, schools, physicians and service providers.
• Travels to locations, agencies, and resources to arrange home-based services for children and families.

Education

Bachelor's degree in eligible fields

Other equivalency with RN or HS diploma and required credits
