The role provides case management, budgeting and resource coordination within a transitional housing program, collaborating with staff to deliver services and maintain client records. Strong communication and confidentiality are essential.

Qualifications

• Bachelor’s degree in social work or related field; Master’s preferred.
• Minimum of three years’ work experience in residential setting preferred.
• Experience providing case management services.
• Two years’ experience with women and children desired.
• Proficient with email and Microsoft Office.
• Excellent written, verbal, and interpersonal communication skills.
• Team approach; proactive in sharing information with others.

Responsibilities

• Review potential members to determine placement fit.
• Administer assessments.
• Provide case management aligned with ISP and Transition Plans.
• Develop long and short-term goals with members.
• Maintain client records per regulations within 48 hours.
• Use motivational interviewing and trauma-informed approaches.
• Coordinate with community partners and resources.
• Attend staff meetings and funder reporting.
• Manage scheduling for life skills classes and events.
• Track and report member outcomes monthly/quarterly.
