Job summary

Responsibilities

• Identify and maintain resource lists for clients, including healthcare options.
• Provide tailored referral support for healthcare resources.
• Coordinate health screenings and education materials.
• Collaborate with partners to access services for families and youth.
• Coordinate data entry in IRC systems and participate in meetings.
