Job summary

Health Connect America in Alabama seeks a Family Development Specialist to support clients and families by developing and implementing treatment plans, delivering therapeutic interventions, and connecting them with essential services in home and community settings.

You will collaborate with a treatment team, maintain thorough documentation, recruit new referrals, and uphold COA standards while providing 24/7 on‑call support as required.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care with the treatment team.
• Provide individual, group, and family therapy and skill-building per the Treatment Plan.
• Attend weekly Treatment Team meetings.
• Link clients to services and resources in the treatment plan.
• Liaise with other professionals and community resources to meet client needs.
• Maintain accurate documentation of all client interactions and progress.
