Job summary

The Central-Jersey-Family-Health-Consortiu is seeking a Program Support Specialist for the Family Connects home visiting program in New Jersey. You will enroll families, connect parents to the program, and document data for the eligible birth population while supporting nurse referrals.

Responsibilities include coordinating nurse schedules, recruiting families, engaging hospital staff, and following up with families after visits. Strong bilingual communication and data-entry skills are essential.

Qualifications

• Education: Bachelor's degree in education or health related field required.
• Experience: non-profit, maternal child health and education preferred.
• Fluency in English and Spanish; data entry and attention to detail.
• Ability to collaborate in a team and work independently to meet deadlines.
• Experience in patient or client environments serving underserved populations.
• Excellent interpersonal skills with diverse groups.

Responsibilities

• Coordinate scheduling of all nurse assignments and manage client database.
• Assist with client recruiting and engage families for nurse visits.
• Establish contact with hospital birth centers and staff; attend meetings and trainings.
• Schedule nursing visits via phone or in-hospital visits as appropriate.
• Conduct post-visit follow-up calls and provide re-referrals when needed.
• Handle appointment confirmations via text messaging.
• Serve as primary public contact and reschedule visits as needed.
• Participate in regular Family Connects meetings and trainings.
• Complete other duties as assigned by Nurse Manager and leadership.
