Job summary

VNA Health Group in Middlesex, NJ is seeking a Family Support Worker (FSW) to provide in-home support to families, promote healthy child development, and strengthen family dynamics. The FSW builds trusting relationships with parents and caregivers and helps develop personalized goals.

Responsibilities include home visits, linking families to resources, and coordinating with service providers while maintaining records and ensuring a client-centered approach throughout New Jersey.

Qualifications

• High school diploma or equivalent required.
• At least 2 years of experience working with children and families.
• Bilingual in English and Spanish is required.
• Valid driver's license and auto insurance.
• Willingness to travel throughout New Jersey.

Responsibilities

• Outreach to families referred by Connecting NJ or a Service Provider to introduce services and determine eligibility.
• Utilize the Family Resilience and Opportunities for Growth (FROG) Scale with parents to identify strengths/risk factors to support the family throughout home visitation services.
• Conduct home visits primarily in the family’s residence, ensuring each visit lasts a minimum of one hour and follows a family-centered, strength-based approach.
• Support healthy parent-child relationships by helping parents strengthen emotional connections and improve their parenting skills.
• Addresses gaps in care of clients – effectively work with individuals from diverse backgrounds in reducing cultural and socioeconomic barriers between clients and health providers.
• Collaborate with families to set achievable goals, create action plans, and monitor progress toward those goals.
• Observe and assess the normal growth and developmental milestones of the target child.
• Maintain accurate records and documentation as required by agency guidelines.
