Job summary

Wellpoint Care Network in Milwaukee is seeking a Community Connect Coordinator to reduce social isolation by linking people to their communities and resources. This role focuses on improving parenting skills, health knowledge, and self-sufficiency through outreach, coaching, and advocacy.

The ideal candidate will have a High School Diploma or GED, experience with vulnerable families, excellent relationship-building abilities, and strong computer skills.

Qualifications

• High School Diploma or GED required.
• Experience in relationship-building and service coordination in social services.
• Experience engaging with vulnerable families with children in home settings.
• Knowledge of local neighborhood services.
• Ability to work with communities and families; strong problem-solving skills.
• Proficient with computer software including Microsoft Office, Excel, Outlook, Teams, ADP.

Responsibilities

• Works with staff and clients to connect with community partners and strengthen family well-being.
• Collaborates with Wellpoint staff and families on program models and access to services.
• Assist families with scheduling, transportation to appointments, and accessing community resources.
• Identify and remove barriers to accessing services for families.
• Provide resources and guidance to navigate community opportunities; refer to community agencies.
• Greet new Wellpoint clients with welcome materials; support staff with referrals.
• Review requests for financial support and advise leadership; document family needs.
• Contribute to Five Pillars of Stability assessments and program needs analysis.
