Job summary

Health Connect America is seeking an Integrated Care Coordinator to support clients with behavioral and physical health goals. You’ll oversee care planning, coordination, and monitoring across treatment teams, primary care, and community resources.

Responsibilities include developing person-centered care plans, coordinating services, following up after hospitalizations, and maintaining timely documentation to meet regulatory standards.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis, and may be accumulated before or after degree completion.
• For care managers serving members with LTSS needs: Two years of prior LTSS and /or HCBS coordination, care delivery monitoring, and care management experience, in addition to the required cited above. (This experience may be concurrent with the two years of experience working directly with individuals with behavioral health conditions, an I/DD, or a TBI, above.)

Responsibilities

• Oversee care planning, coordination, and monitoring for assigned clients, supporting both behavioral and physical health goals.
• Conduct assessments and develop individualized, person-centered care plans based on client needs, health goals, and SDOH.
• Coordinate and monitor services across treatment teams, primary care providers, and community resources to ensure seamless care delivery.
• Facilitate follow-up services for hospitalizations or transitions of care to support smooth continuity.
• Monitor progress, verify enrollment and payer status to ensure access to services.
• Maintain timely documentation of care coordination and interactions in line with standards.
• Support clinic operations, including scheduling, reminders, vitals, charting, and collaboration with Nurse Practitioners.
• Participate in marketing initiatives, outreach to new referrals, and education of prospective clients to promote engagement and enrollment.
