Job summary

AMHC seeks a dedicated Nurse Care Manager to join our integrated outpatient team, bridging physical and behavioral healthcare. You will collaborate with behavioral health professionals, primary care providers, and community partners to ensure coordinated, high‑quality care and wellness support.

The role involves coordinating medical and behavioral services, guiding transitions from hospitals and crisis settings, and leading health education initiatives.

Qualifications

• Bachelor of Science in Nursing required.
• Active RN licensure in Maine (state listed in Requirements).
• Minimum of two years nursing experience preferred, with behavioral health or integrated healthcare experience preferred.
• Valid driver's license and ability to travel within the service area.

Responsibilities

• Coordinate medical and behavioral healthcare services with multidisciplinary teams.
• Support transitions from hospitals and other treatment settings.
• Conduct ongoing assessments of medical and psychiatric needs through chart reviews and provider collaboration.
• Coordinate care with primary care providers, specialists, pharmacies, hospitals, and community resources.
• Review medical records, labs, medications, and treatment recommendations to promote continuity of care.
• Provide education and support to clients, families, caregivers, and staff on wellness and preventive healthcare.
• Develop and facilitate health and wellness groups.
• Participate in quality improvement initiatives and integrated care efforts.
• Provide nursing consultation and on-call support to residential and crisis programs.
