Job summary

Kennedy Krieger Institute, Inc. in Baltimore, MD, is seeking a Planning Coordinator I to provide family-centered coordination services for IDEA Part C Early Intervention infants and toddlers and their families.

The role focuses on developing and revising IFSPs, collaborating with families, and coordinating multidisciplinary support within BITP, while ensuring timely documentation, referrals, and transition planning to support child development.

Qualifications

• Associate’s degree required; 2 years of experience may substitute at leadership discretion.
• Bachelor’s degree preferred.
• Previous work in the field of disabilities, ideally with children and their families, preferred.
• Previous case management experience preferred.

Responsibilities

• Provide home- and center-based family-centered services to Part C-eligible infants and toddlers and their families.
• Attend and participate in BITP Part C eligibility determinations and family assessments to develop the IFSP.
• Complete and maintain monthly contacts with all families and document billing promptly.
• Effectively monitor required timelines for BITP parts B and C and maintain paperwork.
• Monitor developmental milestones and family outcomes.
• Complete intake referrals, schedule evaluations, and order assessments as needed.
• Contribute to transition planning and assemble transition documentation for Part B IE P meetings.
• Serve as part of the early intervention team, collaborating with providers and families.
• Make referrals and collaborate with outside agencies.
• Provide support and information to promote family collaboration and self-advocacy.
• Participate in program meetings and trainings to improve quality and growth.
