Qualifications

• Associate's degree in human-services or related field is required; a bachelor's may be preferred.
• Two years of experience may substitute for this degree per leadership discretion.

Responsibilities

• Provide home- and center-based family-centered services to part C-eligible infants and toddlers and their families.
• Develop and monitor the IFSP and collaborate with families to determine concerns, priorities, and needs.
• Attend BITP Part C eligibility evaluations and family assessments to develop the IFSP.
• Complete monthly contacts with families via phone, home visits, or agency visits and document billing promptly.
• Monitor timelines for BITP Parts B and C and maintain required paperwork.
• Complete intake referrals, schedule evaluations, and order assessments as needed.
• Support transition planning, gather documentation for Part B eligibility/IEP meetings, and coordinate extended IFSP when needed.
• Work with the early intervention team and external agencies to maintain referrals and best practices.
